I'm reading Richard O'Kane's Clear the Bridge,and according to the author,the engines couldn't be ran at flank speed for long periods of time because they might break.Kind of like war emergency power in an airplane.That would be a mod idea if it could be done.My guess is that going full ahead wouldn't have any adverse effects on the engines.
The Tang has reached depths of 650 feet so far in my readings,and at that time the Japanese didn't have any depth charges that could get close enough to harm the Tang.It's sad that a circling torpedo done the Tang in.It's a miracle that anyone survived the hit at all.

In the case of fleet boats they usually never needed to run the 'engines' at flank to achieve maximum speed. They were diesel-electric and had somewhat more engine power than electric power. It's been a while since I've read that book, but I think O'Kane was pushing the engines to the limit to maintain maximum speed and charge the batteries, something a U-boat cannot do because of its direct drive powerplant.

Thus, in the case of U-boats, max speed did require max engine power and anything used to charge the batteries took away from that speed. I do agree, though, that the engines should have a small chance to sustain damage over hours of running ahead flank. Furthermore, I think that when 'overcharging' the diesels, something I believe is possible in real life, instead of the only penalty being mega fuel guzzling the fuel use should be realistically proportional to that increased output (maybe 10% more power and 20% higher fuel consumption?) and instead the chance of mechanical failure should be greatly increased.

Now, as for whether or not any of this is possible in the game... I dunno.:hmmm:

I'm thinking it could be modded,but like you said,I'm not sure.O'Kane stated that it was possible to make an explosion possible when overcharging the batteries.It was over my head,but something about highly volatile gases being created,that could easily ignite.
Another thing that even made me cringe a bit,was when they had there first encounter with a DD,and how close the tin can got,and the cat and mouse game that went on.The Tang had a leaking problem on a torpedo tube and had to run pumps to keep the flooding down,which gave the DD fixes where the Tang was.One had to have nerves of steel to be a submariner.

i personaly agree i have a cousin in the navy and it is not just nerves they had to be extremly fit

but i also watched an australian documentry on dvd called submariners
about the collins class subs and crew
