Any other ships that have had as bad a run (in terms of names) as the Porter?

In the article it says the whole crew was arrested. What happened to them? Were they court martialled?

HMAS Melbourne was close. It ran over two friendly warships during manouvres in peace time.

Seems to me that HMAS Melbourne wasn't unlucky - the destroyers assigned to guard her were! In each of those cases it looks like the escorts were clearly at fault based on the rules of manuevering at sea.
