AFAIK this is NOT gun damage!
I don't know the name of that vessel, but I think it was hit by debris when the ammunition ship USS Mount Hood (AE-11) blew up in Seeadler Harbor, Manus Island in November 1944.
There was almost nothing left of the ammunition ship, and the only survivors where those that were not aboard at the time of the accident.

The story linked on the "Bones" action was enthralling. Tough little escort ship.

Now this makes sense. My first impressioin was that the ship had been shot by a giant shot gun !
