I have the Direct2Drive version (1.4 patch) and have had that too, so nothing to do with STEAM. No idea about cause, but now I make sure to save before entering port to refit, then wait a minute or so for progress to indicate the start of the load before clicking the refit anchor again. Then time compression and wait for all tubes to be loaded before saving again. Best I recall the first time I had the glitch I had saved and exited before the tubes were loaded, and when I reloaded the save I got that glitch.
