You can also get that blank paper in the Data / Menu / Skins / American / Gui / Layout / radio_messages.dds file. In the FotRSU mod, in the Support / Research folder is a "SubmarineActionReportForm.pdf" file, of which I did the same with a copy of an actual report form from one of the
Submarine War Patrol Reports, which are included as part of the data collected, along with the weather, plane sightings, etc. Use a couple of the actual filled-out forms in some of the War Patrol Reports as a "model", if you use the Action Report Form. You could also "model" a report after a sub-section of a War Patrol Report, like Bubblehead1980 does. Pepper some radio messages in your report like 4H_Ccrashh, and make a pdf of the whole set... You of course, have to fill it out quintuplet with good, clear carbons so that one goes on file, one to ComSubRon for endorsement and up the chain, one to ComSubsPac for off-site record keeping, one for yourself, and the fifth for when someone loses their copy...
