I believe this is a stock bug that arises sometimes. You can see the ship, but it doesn't show up on the map or notepad, and you can't lock the ship in your UZO or periscope. As mentioned above, seems to happen more in poor visibility or with several other ships around. Usually resolves itself as you get closer. Usually.
