seems the saving with an open tube cause it but thats a wild guess

It looks like you saved whilst submerged.
If that is the case.....your asking for all kinds of trouble.
As for an 'open tube' the torpedo doors close automatically after the torpedo is ejected from the tube. |
All together now Never Never save the game when Submerged
