I've run into the same problem. "Realistic crew" seems to be ignored if checked. 
I went back to version 3.0 and the realistic names feature works. Unfortunately, I seem to have lost my version 3.1.1. Can someone provide me with the sh3cmdr.exe for version 3.1.1 or the entire 3.1.1 file?
