As soon as my first commander retired i continued with a new commander. I saved the game mid-patrol and exited the game. Then a window in SH3Cmdr pops up saying that "index out of border(s)". That's about it.

"Index out of bounds (11)" will be the error.
This will occur with careers originally started pre R2.3, or if you attempt to start a
new career using the
same name as a career originally started pre R2.3. Why? Because SH3Cmdr R3.0+ dropped internal handling of such careers.
Solution? Open SH3Cmdr.ini (located in your SH3 Commander folder) and remove the Commander name from the [CAREER DETAILS] block. When you next select him in SH3Cmdr, new details will be chosen for him (which is, I guess, what you want anyway considering you are starting a new career).
