I think so too, it was a mine, but I blame TC or SH3 commander not stoping it on time  as the H.I. shows the hit was not criticall yet

Dont blame the most fantastic tool for SH3 ever for it. In the end its the fault of SH3 alone (and maybe a weak/slow system).
It happened to me too. But ages ago. Mines wont destroy your complete hull, but the damage they do to you is so extrme that you mostly die instantly.
I even survived a mine once. It was with GWX 1.0 or maybe 1.03. My bow compartments were totally ruined. All Torpedotubes were gone and the whole crew in the bow are too. I had heavy flooding, but stopped it just in time and made it back to uh... I guess it was Brest...
Mines suck! They can ruin the finest career in no-time!

You survived only because TC worked fine I bet, or u weren't at TC at all. 84 % of H.I. is a peace of cake to handle with damages, but not when TC goes on 256 faster real time and water flowes in on an eye blink!
And my system is average (AMD sempron 3400+, 1GB of RAM, Nvidia GF6600), this is definetely something with SH3 commander's TC settings malfunction on critical hits
