I've had the same error plus,envsim.act , and 3 others. all different.
CTD always happens to me always mid ocean at peri depth and just getting ready to fire torp or useing < > keys to look at ship after hearing WO say " ship spotted " I can be in harbor and look at all other ships and no problem.
