Usually it was damage/grounding (s-boats, mostly).
USS Darter grounded under enemy fire..but all her crew was safely rescued by USS Dace.
Fleet boats never refuelled at sea ( never seen a photo showing a refuel at sea )
U-boats are a different thing, in the south atlantic and indian ocean they usually were refuelled by surface ships like blockadebrechern and even auxiliary cruisers ( the mighty KMS Atlantis was lost while refuelling U-126 )
