I been thinking it can be something to do with that in the picture that is in the log of your u-boat where you promote crew, do not have the repair crew compartment as in the other crew/u-boat management screen. And basic the game somehow forget to put him back, since he do go there at your will and not the game, since if you right click to move your repair crew then he do not move from or to repair compartment, like the pettyofficer that have a repair qualification.

I remember back in 2005 someone here uncovered a problem with the fatigue modelling. I can't recall the fine details, but I know it was some problem between compartment 1 (radio room) and compartment 11 (repair), seemingly an internal bug of some type.
I am betting that this problem is somehow related, and could be put down to something as simple as internal confusion between the 11 and 1. This sounds funny, but as anyone who has tried to mod the torpedoes knows, any torpedo in slot 11 is ignored and the torpedo from slot 1 is used instead.
Anyway, if I manually tweak the crew files, I can force the problem that you guys are getting, but that doesn't really help.
As someone said back in mid-2005, it certainly looks like that originally there was to be an officer slot in compartment 1, but at some stage it was removed, but the associated assumptions were not corrected.
I'm not out to fix that, just provide a way to get the wayward man back.
@Nightshadow: Are you saying that your file snippet is taken immediately after an officer goes MIA? With no-one in slot 11, it doesn't look like it (at worst there should be crew details even if NrComp=-1). I really want to see crew extracts taken immediately after noticing the officer is gone.
