Only thing I can think of is;
Are you completing all your objectives and how much tonnage are you getting?
Getting a medal seems to be tonnage related and how many objectives you complete?
As for the sub upgrade, it might be the same deal as the conning tower changes - get a lucky role and you get an upgrade - if you don't, your stuck with what you've got.
