In my last patrol (which was also the first of current career) I too returned right into the Kiel dock I started from. (felt just like the Das Boot scene) And I didn't receive any renown for the ships I sunk, a small freighter and V/W class near Hartlepool. Only renown for reaching and completing patrol grid. I am not sure what caused it. As I didn't change anything on my Uboot before 1st patrol other than add extra crew (does that also qualify as 'upgrading' before 1st patrol?). But i also had SH3 crash on me mid-patrol, so had to restart from a savegame. Maybe that screwed stuff up.
