It appears officers go from Ensign, to Lieutenant Junior Grade, to Lieutenant. But I don't know if the game displays the 'junior grade' for Lieutenants.

I'm pretty sure that the game does display the 'junior grade' for Lieutenants. In fact since installing TM 1.6 all my officers have been promoted to LT Senior Grad (took me about 7 cruises, depending on whether the officer was an Ensign or a Lieutenant Junior Grade).
