Promotions in SH3 are linked with Renown and not tonnage. The renown you collect is a function of the realism settings you're using (i.e. if you play at 50% realism you will collect the 50% of the possible renown you can etc). Therefore by playing 0% realism you collect no renown. Hence no promotions as well
