It'll be an engine malfunction, kindly supplied by SH3 Commander!
I had the same thing some time back though I've since disabled that option because of the irritation factor. It might be nearer to reality, but things like that happening annoyed the hell out of me!
It must have been murder for the RL commanders, I'm glad we can turn them off!
