Well, a combination of two things "messed" your career: 1. the way SH3 behaves when reverting to previous saves and 2. the way you reacted when met with SH3's behaviour.
It's well known here (at least amongst the older Kaleuns) that SH3 gets a tad bit confused when you revert to a previous save. When you revert to a previous save, SH3 will still display some "current" information, including (but not necessarily always) bases and dates (that's the funny one to see).
You need to ignore this as SH3 should get itself back on track when you actually start the patrol (like it seems to have done for you - it said "St Nazaire" as the start base which is a carry over from your dead patrol, but then correctly started at Kiel, which is where you originally started the same patrol from, at least that's how your comments read to me).
If you didn't get the "Base changed" message during the "re-do" patrol, then you should have docked back at Kiel (for example, this time your patrol may have been shorter, thus not tripping the base change date).
Either way, I doubt your career is "messed". You just need to revert to a previous save, start at the base that it takes you to, and plan on ending AT THE SAME BASE. Then watch for any ingame messages that tell you that your base has changed and follow accordingly.
Note, in case it is not clear, St Nazaire is NOT your base until you get the "Base changed to: St Nazaire" radio message.
Base changes are a PITA in relation to save games, as anyone who's gotten the repeating "Base changed to:" message will attest.
