8:5 just refers to the screen dimensions. ie. 8:5 translates to 800:500 or 1600:1000 or anyting of the same ratio. So a 800x600 image (8:6 or.. 4:3 if you like) won't fill my 8:5 screen. I'll get black lines down the sides, unless the graphics are stretched by my graphics card (which distorts, not giving you a true image). Most craphics cards drivers are set to stretch by default because your average user won't notice (or mind the slight distortion).

The refresh rate is seperate. They're often fixed (at say 60Hz). If your game is running at any given fps that isn't 60 or a multiple of 60, you'll notice the screen tearing as you spin around on the bridge. Jagged horizontal lines because the fps and monitor refresh rate dont match. V-Sync gets rid of these.
