I've redownloaded the mod folder (hats+insignia) and my Lieutenants and Lieutenant (J.G.)'s are wearing their nice hats, but still have the Ensign golden bar insignia on all of their collars.

I'm running the game with the graphic settings maxed out, and in some cases driver override on the AA and AntiS filters.
