"von/vom" means "of" or "from". The difference between the two words is grammatical.
When you have only a year you write:
"von 1939-1945"
if you have a more exact date you write:
"vom 01.09.1939-08.05.1945"
To fit the given space you can write "Position" or "Standort" instead of "Ort". The best sentence is "Angabe des Ortes". That is the real sentence in the German diary, but to long for the space.
