There must not be many kapt'ns here who didn't one day sink a neutral...
I don't mind crew management myself but if it is too much of a pain, you can use SH3Commander to totally disable fatigue. Another tip is to alter the "main" file of your cfg folder in MyDocuments/SH3 by setting a lower 3Drender max value. At any TC above this, the fatigue don't work (but also so does the resting..) I think each of us adjusts this fatigue to his own idea.
For the realism, it's up to you when you feel comfortable, I remember it took me a lot of time to fly 100% in IL2/PF with complex engine management, no padlock, no minimap path, etc... Now that I do, I enjoy it a lot more...
For SH3, it also took me a long time to go to full manual TDC, I always kept the WO help "just in case"... Once you get it, in fact you realise it was not that difficult and it makes the experience much better, if you are a die hard simmer.
