You have hit mines when leaving port
. In the map screen on the upper left corner their are 3 pulldown sheets one of them has ports showing where mines are laid.
As for the ! icon i wouldn't know, probably something to do with fatigue. I prefer to turn off fatique altogether using SH3 Commander 2.6.
