Depending on what version of the game you're running will determine what happens.
For one, attacking a neutral will have you 'flagged' as attackable by that country for a period of time afterwards. For example, you attack a US vessel in 1940, and after that, for a while, US ships will fire on you if they detect you.
For another, in the stock game and in GW, neutral tonnage sunk is
deducted from your renown for that patrol (and you don't even wanna know what happens of you sink a
friendly ship!). I'm not sure how NYGM handles it, as they don't use renown but work off of straight tonnage sunk.
Supposedly, if you sink too many neutrals, the Kriegsmarine will dismiss you from command and the service.
