I doubt this one test will be enough to spark a US led war, but it is one more step on the road towards one, I wouldn't be surprised that when the DPRK tests this nuke (there's no way it's an if) , the ROK requests that it be allowed to restart its nuclear program as a MAD defensive method, the US (of course) will say no, but ROK will do it anyway...China will probably stage some major military movements to discourage Japan who'll probably send a few more ships into the sea of Japan.
Then its back to the negociating table until Iran distracts the US again, in which case Kim will get jealous and test another Taepo.


Just looking up the possible test sites for the underground nukes, one of them looks to be very near Musudan-ri, the other looks to be north of Pyongyang, closer to the Chinese border than Pyongyang though...looking at that area in Google Earth it is possible to see an underground base...here, I'll get a screenshot:



It's no simple mining facility, as SAM and AAA sites are just about visible in the hillsides around the area...so it's something important, that's for sure

(Third edit) Looking at it though...would you detonate a nuke there? I certainly wouldn't, look at the water sources on the mountain there...not that the DPRK would care about such things admittedly...
