The "cannot initalize 3d" thing occurs when your graphics card decides it cannot support the resolution and refresh rate asked for in d3d9.ini. Windows display properties will give a list of supported display modes which are determined by the graphics card and monitor talking to each other like two drunks in a bar. The resolution and refresh rate in d3d9.ini has to be something from this list.
Setting Refreshrate to false turns off this part of the check and can help.
However it looks like your monitor has a slightly different aspect ratio (16:10) to the one the mod wants (16:9), or possibly just not enough pixels. It is sometimes possible to set up custom display modes to work round this but it isn't straightforward.
i would look for a different widescreen mod such as WideScreen MaGui, which is aimed at a less ambitious 1366x768
