There's no easy fix for this that i can think of, other then isolating what boats are available, so the game has no boat to send you to as an upgrade. This means either
a.) removing all other boats from play (ive tested this, and it works)
or
b.) changing the "UpgradeClass="variable in EVERY submarine.upc file to be the same number, so no boat is considered an upgrade (This is untested )

although this thread is 12 years old, it is still relevant, as players ask the same questions as Ducimus and Razark (and me), et.al.
over the last year or so, i have been tracking refits and what it takes to get a conning tower refit on a US Boat in SH4. I would like to thank Propbeanie for being my sounding board during this project. Here is what i have found:
Conning Tower Refits (referred to as Refits from here on out) are mutually exclusive with Boat Upgrades, which present the player with an entirely different and newer boat to drive.
Refits seem to require the following conditions. These have been tested over dozens, if not hundreds of missions.
- all boats must have the same Upgrade Class in the USBoat.upc file.
- the player must score a mission rating of either 1 or 2 in his last few missions. Rating of three is NOT good for refits. (do not yell at me, it's not my logic. lol)
- the player must sustain damage to his boat in two consecutive missions while achieving a mission rating of 1 or 2 for each mission.
