Most likely not related to your issue, but just an fyi, do not install SH4 - Steam or otherwise (or any of the other SH games), in the default C:\Program Files (x86) folder that Steam is in (also by default). If you have multiple, usable hard drives (not including a system back-up partition), you can create a new "Library" folder off of the C: drive, and you would then be able to apply mods without Windows interfering in your efforts. Windows "protects" the Program Files folders, and will roll back any modding attempts.

As for the "paused" game, that can come from a number of sources. First of all, make certain that the DirectX v9.0c files and the Windows Media Codecs are installed along with the Silent Hunter installation. Even if you have DirectX v12, it does not have all of the library files required of an older "legacy" game such as Silent Hunter 4.

Another thing, is that it being an older game, the computer has to be properly set-up, which includes the display, such that if you have a "native" resolution 1920x1080 LCD display monitor, you set the Windows display driver to that also, and then set the game to show the same. That would be the Graphic Settings from the Options menu item on the Main Menu in SH4.
