I believe that mod was created long before the mods that TWoS is based on were released in their final version. Nowadays you won't meet many players here still using it.

sh.sdl is just the archive where all the SH5 sounds are listed together with their acoustic properties (volume, audible range, pitch, doppler effect, looping, etc). Any new sound file must be logged there for the game to be able using it, but the fact that a sound is in sh.sdl doesn't necessarily mean that it will be used. For a sound to be actually played, it must be called by a script and/or by a controller that will trigger it when certain conditions are satisfied. It is well possible that some stock sounds that you replaced with the ones from the DBSM Speech Mod are non longer used in TWoS because the scripts and controllers which called them were removed or modified to use other sounds, but you should ask Vecko on that.

Yes, as a beginner the chances that you spoil your game by adding random mods are rather high.
My suggestion is that you start playing and enjoying TWoS as it si "out of the box", while keeping an eye on the forum. Once you begin understanding the game and its files, you can start customizing it according your tastes by gradually adding some other mods to your game.
The TWoS package contains many optional mods that are perfectly compatible with the main mod. Moreover, there are some recent small mods by other modders which were created keeping TWoS compatibility in mind, so you know that using them is more or less safe.
Conversely, most of the mods of the past have been already customized and merged in TWoS, while the ones that have not been included in it, have been discarded for a reason. Unless you know exactly what you are doing, messing with them will be equal to searching for problems
