Here in Finland the court would (on request of defence or prosecution) order the suspect into forensic psychiatric evaluation. These are carried out by Niuvanniemi hospital, a state mental hospital nearby Kuopio, Finland. If found to have been 'without understanding' at the time of the crime and also found to be danger to the public, this same institution would likely be their home for rest of their lives.

Here in the UK the defence would first make the plea backed up by their experts then the prosecution will put forward their experts and a jury will decide which one they believe.
