You are doing everything ok.
I think the problem is that you are possibly losing the lock on the target.

There is a lock/break setting in TDW's Options File Editor Viewer.
There is also a setting that will tell you the type of vessel you are looking at.

If the seas are high and keep rolling over the periscope you will lose the target.
If you are trying to track the target through the periscope over a number of degrees in high seas then you are constantly hitting the space bar to maintain the lock on the target.

For example when you first pick up the target in the periscope it might be 80 degrees or more away.

What you should be able to do is lock the target with the space bar. Identify the target through the recognition manual or by using by using the identify icon and send the mast height to the TDC all the time keeping the target locked.

Get the periscope's horizontal line on the targets waterline and pause the game.
You should now be able to use the stadimeter to bring down the line to the top of the targets flag.
Clicking the left mouse button should release the stadimeter and the range should have changed in the TDC.
