the (stock) game did that to me once. I was operating out of Lorient for most of the war; just before the XXIs become available I transferred to the 11th out of Norway. One of the first patrol grids they assigned me was BF61(or 65 I don't recall exactly), but what I do remember was it was the waterways right next to ...Lorient!
I entered the A/O and began my patrol. It did not update that I even made it on station. I swam around for the required 24hrs and (since nothing happened) I returned to base. In failure. Mission not complete.
Never did that one again...
