The really big expense is the surrounding bureaucracy which is immense and extremely unwieldy. There is a lot of "pork" in the business of government and a lot of obstacles in the way of reforming the situation. Take, for example, the current problems in the US Veterans Administration: While there are a lot of calls for the resignation of the Secretary of Veterans Affairs (most vociferously from the opposition party; it is an election year, after all), the sad truth is the upper-level and mid-level officials in the Veterans Administration are protected by numerous laws and regulations so that even if they have failed or are derelict in their duties, there is little or no possibility of removing them from their post as a means of changing the system. Its rather like trying to kill cockroaches cutting off their heads -- the body still keeps moving and lives on...
