EDIT: And, yes, the Auto-Reload was on.

Your set of rules would allow you to reload.
If every other option as mentioned by others wasn't the culprit (crew fatigue, silent running and auto load button) it might be a bug.
PS_ I'd test and see what happends, if the torpedoes reload then continue with the career as per your rules. If they don't, and you're sure non of the mentioned possibilities are the cause, end the career and start over.
