I can't agree that humans are better at determining targets. Drones target recognition is only as good as the humans that either design it or sit at the remote control unit. No difference at all. Blue on Blue incidents have happened since we started putting some distance between combatants with ranged weapons, meaning they go back to the days of bow and arrow. Will it get worse than it has already been? Ultimately I don't think it matters until we get a Skynet situation.
One of the keys to good contingency planning is having a clear understanding of the risk profile. That profile also needs to include the likelihood of a risk becoming an issue.
