"Never save game when submerged.." I've done it many times without a problem so I wouldn't say never do it. If it is indeed a hard coded left over from SH3 it must be a random event.
The game certainly suffers from some sort of "save game" problem which no one seems to have uncovered. Too avoid the agony of losing a career I save a lot so I can at least recover somewhere.
