Granted, that is where I have seen the bug the most (outside), I have had the bug appear randomly (on rare occasions) when I load up TDW's Torpedo Tutorial mission to get around the stupid Echolot bug. It can happen inside the boat, too. It is much less likely to happen, but I have observed it. It doesn't happen with the Mighty Fine Crew mod removed.
