I believe this happends when you install a mod while your on patrol, which will cause you to stay in pause mode. Happend to me a while ago.
Not sure if anyone else has this problem but I notised if im on patrol in my carrear and my ship is damaged by a depth charge or a destroyer hitting me with its deck guns, if I save my game and try to reload it my game makes a very loud hiss/air sound. Kinda anoying but it makes me be more carful when on patrol :P
