I started with a new installation of SH5 and testing step by step, i discovered that if you, like me, have a SH5.exe that is not the "original" and you patch the volume of hydrophone with the TDW_SH5_patcher (last option there) the problem apear. Solution is to put it again in "false" and problem solved.

Do you have steam version or DVD version of game? If you have steam version I can see how that could happen.
