When you look at the files in SH3 Sound folder (open in winamp and right click => track info) you´ll notice they´re stored in many different sound formats.
Some are 44100, 16Bit mono, others are 22500, 16bit stereo and so on.

Examples:
Creaks_Level1.wav = 44100 16 bit stereo
Echolot_ping.wav = 22500 16bit mono
P01_&Big_Shell_water_explosion.wav = 44100 16bit mono
...
The stock game does not use an unique format
I had similar problems like markcubed with every soundpack i downloaded, they always crashed my game.
The stock game runs fine with that mixture of formats, i see no need to change all soundfiles to mono. Instead i tried to keep all sound files i use similar to the default game ones (same format)

So i created a little personal soundpack and changed the format of the files to match the ones they had in stock game. => no more crashes.
You can easily do that for yourself, just compare every single soundfile with the one in the game, if there´s a format mismatch (like in many soundpacks) change it with the windows build in audio recorder to match the file format in the unmodded game.

If you like you can download the little sound collection i use, i promise you it won´t crash your game. ;-)
Just install over an untouched 1.4 installation (or use the mod enabler) to make sure no other mod is causing the crashes.
There are few speech files in too, i use a german game setup.
I could upload it on rapidshare, if markcubed or someone else is interested.
