Don't get the Steam one! You can buy a disc-copy for about £5 on Amazon, don't think you can use the exe patcher with the Steam version.
Yes it's worth it. It needs a lot of mods, but it's worth it. Have a look at Trevally's recent 'Spy Mission OHII' thread (in this forum) for some screens and an example of one of the newer campaign missions.
