Changing the speed will change the positions of the numbers, so if you match up for slow and set them to fast just before firing it will go astray, especially at range. This should be fairly obvious though, you'll see the numbers on the track move in the TAI when you change the speed so it'd be hard to miss. If you're selecting spread fire, make sure you don't set the spread angle too wide and zoom right in on the TAI to check both will hit.

It's no good just firing when the triangle icon goes green, those shots are always way off. You need to line the numbers up on the track. Alternatively the torps could have been spotted and successfully evaded - it does happen. Take your shots from 1000m or less to reduce the enemy's chance of evasion, and have your torp doors already open or the delay can put you off track as well.

Or just lock, aim at the middle of the ship, unlock and fire. Crazy, but it works.
