I use it for screenies only, and only after I have shot torps. If there are no enemy around and I want some screens of my sub leaping through the waves, then I will spend a while up there getting the right angle.
My first 3 patrols I used it after torps launched and before they hit.
Usually now, I watch through my scope until I see an explosion, then pause game, then freelook for a good screenie, then back to my sub and turn the time back on.
While the temptation is strong to glance up above while I am being pounded with depth charges, I have been able to resist so far.
92% realism is fine for my 1st career. I will go to 100% for my next career once I die or the campaign ends.
