I can't speak for all the various ways the game has been sold. Hard boxed CD's, Internet downloads/addons, Golden Editions. I'm guessing they all would have the same basic files as my original 1.0 boxed version.
You need to use Windows Explorer and find your root folder of the game. Every computer OS is different in where you put the game when you first installed it. For Windows XP the default installation would be C:/ProgramFiles/Ubisoft/Silent Hunter Wolves of the Pacific/
In the root folder of the game there are two JPEG Image files for the front and back sides of the Keyboard Card that you are looking for. File names SH4_Q-Ref_Card_Front, and SH4_Q-Ref_Card_Back. Print them off and your good to go.
