But if you eliminate the major population centers with direct strikes, the fallout and radiation will render large areas of the rest of the world uninhabitable, percipitating the death of a large percentage of the remaining population. Furthermore, the resulting chaos and anarchy among the survivors will not only result in even more deaths, but also further destruction of the remaining resources, causing more deaths from starvation and disease, thus easily surpassing whatever you're suggesting in death toll.
