2. AFAIK, no the "vanilla" game does not take this into account. The viewing distance is artificially limited to, IIRC, 9,000 meters or 12,000 meters. Although I think there was a mod that extended it out to 16,000 meters. Not sure how TMO or RFB handled it.
For instances, US submarine skippers first inkling of a nearby ship was smoke on the horizon, which could often be seen a great distance(Japan still used a lot of coal burning ships) and well before the ship actually came into view. The game does not take this into account because you would probably need a supercomputer to run such a sim.

There is a mod on here called "better smoke merchants and warships" its great, gives merchants the thick high rising black smoke and warships the light whispy smoke, really helps with visual tracking and can see them from many miles away.
