Thats a new one, as far as I'm aware.
Maybe try a re-installing SH5, no mods, see if that fixes it.
Then start adding yours mods one by one, see which one causes it (if it indeed happens again).
As for your mods, I'd lose
Environment 3.2, a few people have been reporting a couple of bugs, including myself. Try
SH5EnvModGold instead, Sober swears by it, and it's slowly growing on me too.
As long as you are putting TDWs IRAI in after his New UIs, all will be fine, it's a known conflict.
