I tried to duplicate this. I tried with both the Shkval in the Akula II and the Akula I. I tried to go to different stations in the submarines. I fired multiple Shkvals. And I was unable to duplicate this situation. I had zero crashes.
I'm running DW with only the v1.01 patch, Subroc fix, and Sound-Speed fix. Have you modded anything yourself? I added a mod at one point and forgot I did it.
I put the stock database back and all seemed well with the problem I encountered. I hate to recommend drastic action, but if you continue to have problems, a fresh re-install might be an option. Sorry I couldn't help you more.
