Spotting the enemy is a two-way street. Sometimes you'll see the smoke on the horizon before your crew whereas other times they'll spot stuff you can't make out yet (particularly in hazy weather or rough weather). Just something you have to get used to in the game.
You'll find the same type of thing with sonar. Sometimes your sonar operator will say "no sound contact" but when you check yourself you can hear something etc.
One thing to be aware of; if you get a ship spotted but can't make out anything and nothing on sonar, it may be fishing vessels which use sail and therefore are; a. quiet, and b. small.
Aircraft too can be unpredictable but IMHO that's not unrealistic. I have had occasions where I can see an aircraft crossing our path well behind us or well in front of us and it continues on its merry way without "seeing" us but other times they'll be tooling along in the distance and then bank in your direction (and you'd be wise to immediately crash dive). Do not wait to try and see what type of aircraft it is, it may be too late, get your butt underwater fast. Be advised that TMO allows the aircraft to 'see' your sub underwater for a fair depth (up to 100ft IIRC) so 150ft depth is a good minimum depth to aim for when crash diving (160ft is default for crash dive anyway).
